Which plan do I apply for?

Each applicant is eligible for only ONE plan at any given time. Applicants are required to apply for the plan that corresponds to his or her average number of hours worked per week and his or her age. Use the table below to determine the plan for which you are eligible.

Current EmploymentYounger than 6565 Years and Older
18 or more hours/week Plan 1 or Plan 1A Plan 65+
Less than 18 hours/week Plan 2 Plan 65+
Retired Plan 2 Plan 65+

PLEASE NOTE:

  • Applicants must be actively at work in order to apply.
  • Average number of hours worked per week must be determined by the applicant and stated on Form 1 in the applicable section. Proof of average weekly hours will be required should you file a Long Term Disability claim.
  • It is the applicant’s responsibility to notify HCP of any change in employment status, average weekly hours worked, etc.
