What Plan 1A benefits include:
Employee Life Insurance ($10,000) & Employee Accidental Death, Disease & Dismemberment ($25,000)
Applicants not actively at work in an Endorsing Hospital and not within a 60 Day Window will be
required to submit health evidence in order to qualify for this coverage.
All Group Life Insurance coverage ceases when you retire, leave the hospital or turn 65 years of age.
This life insurance can be converted, without medical evidence if done within 31 days of coverage termination.
If you are disabled and remain disabled for more than 6 months you will not have to pay for your Employee Life Insurance
and Accidental Death, Disease & Dismemberment coverage for as long as you are disabled - as defined in the master policy.
Employee Long Term Disability Income ($1,000 per month)
Applicants not actively at work in an Endorsing Hospital and not within a 60 Day Window will

Optional Life Insurance
You may purchase optional employee life insurance in units of $10,000 to a maximum of $500,000
- Optional employee life insurance (for the employee) is convertible
You may purchase optional life insurance for a spouse in units of $10,000 to a maximum of $500,000
- Optional spouse life insurance is convertible
You may purchase optional life insurance for a child: in units of $5,000 to a maximum of $50,000
- Optional child Life Insurance is non-convertible
Optional Long Term Disability Income (Employee only)
You may purchase additional long term disability income protection up to a total of 65% of your salary, to a
maximum benefit of $5,000 a month (includes the $1,000 Basic coverage).
How do I apply?
- Fill out Form 4 and submit it with your application (Form 1).
- Evidence of good health is required for all Optional coverage. Complete Form 3 and submit it with your application.
