New Plan Enhancements Effective November 1st, 2007

Basic Long Term Disability increases to $1,000 from $800 per month

Basic Accidental Death, Disease and Dismemberment Insurance increases to $25,000 from $10,000

The limit for excess Long Term Disability increases to $5,000 from $2,500
-
Laser eye surgery is now included under the Basic and Enhanced vision care for all plans
-
The $150 per day maximum for semi private hospital coverage under the Basic extended health provision for Plan 1 and Plan 2 is removed.

Additional coverage for podiatrist and chiropodist is now a combined coverage under the Basic Plan 1&2 for $400 per month

New Optional Coverage Optional Employee Life Insurance (convertible)
Optional Spousal Life Insurance (new – convertible)
- Employee and spousal Life Insurance can now be purchased in units of $10,000 to a maximum of $500,000
- No rate change in November 2006. Unit rates are the same as those effective November 1st 2002.
- Spousal Life Insurance rates will be the same as Employee Life Insurance rates.
Optional Child Life (new-non convertible)
- Child Insurance can be purchased in units of $5,000 to a maximum of $50,000
- Unit rates will be effective November 1st 2006
Extended Health Care (all plans)
- Dietician and Occupational Therapist to be added to the list of Paramedical Services as part of the paramedical services group maximums of $400/yr (Basic) and $500/yr (Enhanced)
- Private Duty Nursing now includes services of an RPN as well as an RN
Dental - Enhanced Option Only
- 50% Major restorative services added after the 36th month on the plan with a $500 annual maximum as part
of the overall annual Dental maximum of $1,000
- Major restorative services include dentures: standard dentures including complete, immediate, transitional and partial dentures.
- crowns: standard onlays or crown restorations (paid to full metal on molar) to restore diseased or
accidentally injured natural teeth
- bridges: standard bridges, including pontics, abutment retainers/crowns (paid to full metal on molar) on
natural teeth
- standard repair or recementing of crowns, onlays and bridge work on natural teeth
