If you are a hospital employee who is retiring or already retired and you are under
65 years old you are eligible to apply for Plan 2 health benefits.
Plan 2 Benefits include:
- Extended Health Care
- Drug Plan (pay-direct card)
- Optional Dental Care
Plan 2 Extended Health Care is offered at 3 different coverage levels:
- Essential*
- Complete
- Optimum
*Regardless of your health or when you apply, if you are a part time hospital
employee you are guaranteed basic (or Essential ) coverage.
However, depending on when you apply and where you work, you may be guaranteed

Applicants who submit Form 2 may be considered for the most extensive coverage (Optimum level) at no additional cost.
